Mental Health Counseling Records Stolen in Office Burglary
The Fox River Counseling Center on Bay Shore Drive, Oshkosh, WI, was burglarized on October 23, 2015, and an unencrypted laptop computer was stolen. Highly sensitive data were stored on the laptop, including the medical records and personal information of an undisclosed number of patients. Social Security numbers, details of mental health status, and the results of psychological tests could potentially have been accessed and viewed by the thieves. Other data believed to have been stored on the laptop include patient names, addresses, dates of birth, statements of work capacity, medical diagnoses, and medical histories. In total, 509 patients had their data compromised. Affected patients had visited the Bay Shore Drive Fox River Counseling Center between Nov. 7, 2012, and Aug. 19, 2014. Nursing Home Snapchat Photo Sharing Scandal Uncovered
An investigation by ProPublica has revealed widespread abuse of patient privacy and dignity by workers from nursing homes and assisted living facilities across the country. Nursing facility workers have taken embarrassing photographs of patients and have shared them on social media websites such as Snapchat. Some of the photographs and videos that have been shared by nursing facility workers show physical and mental abuse of patients suffering from dementia. The ProPublica review uncovered 22 cases of HIPAA-violating photo and video sharing that had been reported since 2012, with 35 instances of inappropriate image and video sharing found in total. Some cases involved workers taking photos of naked or semi-naked patients and posting them on Snapchat, while others involved humiliating and degrading videos of patients. Hundreds of Medical Records Found Scattered in Street
The medical records, billing statements, registration information, and accounting records of hundreds of patients of Fort Myers-based Radiology Regional Center have been found scattered in the street following an accident that occurred during transportation from storage. The records, many of which were over ten years old, had been arranged to be collected from the healthcare provider’s offices and taken to the country incinerator to be destroyed. However, when the country collected the records, its workers failed to secure the rear doors of the vehicle used to transport the boxes. The records fell from the vehicle into the street. Paper files containing highly sensitive information were discovered littered on Fowler Street, and had found their way into doorways, canals, and were scattered all over the sidewalk. New Mexico Department of Health Data Breach Affects 561 Individuals
A New Mexico Department of Health data breach report indicates 561 patients have had their Protected Health Information (PHI) exposed as a result of the theft of an unencrypted laptop computer from an employee’s vehicle. The Department of Health employee does not appear to have been targeted, as a number of automobiles were broken into on the same day in the church parking lot where the vehicle was parked. The theft was reported to law enforcement officers, although the laptop computer has not been recovered. An investigation was conducted to determine what data were stored on the laptop. Some of the information was password protected, although patient first and last names, dates of birth, medications, facility unit, and in some cases, medical diagnoses, were also stored on the laptop and could potentially be accessed by the thief. IT Pro’s Security Concerns for 2016 Probed by Spiceworks Survey
A new IT security report issued by Austin-based IT firm Spiceworks indicates 80% of organizations have suffered an IT security incident this year. The company conducted a survey of 200 U.S. IT professionals to find out more about the security incidents suffered in 2015 and to gather opinions on the biggest data security threats for 2016. This year was challenging for IT professionals, with numerous IT security incidents suffered. In spite of this, optimism appears to be high. 71% of respondents said they are planning to increase security in 2016 to deal with cybersecurity threats and next year should seem them much better prepared to deal with security threats. The Biggest Data Security Threats in 2015 In 2015, the biggest security threats came from malware, with 51% of organizations reporting they had suffered a malware attack during the past 12 months.
